All Sports Mastery Basketball Camp Photo Non-Release Form

I do not permit my child to be photographed or videotaped during the All Sports Mastery Basketball Camp held at College Grove Athletics on the following dates:

Child's Camp Session:

- Girls Camp: June 17 - 20, 1 PM - 4 PM

- Boys Camp: June 24 - 27, 1 PM - 4 PM

By submitting this form electronically, I am opting out of allowing the camp to use any photographs or videos of my child for promotional or marketing purposes.

I acknowledge that my child may still appear in group photos or videos taken during camp activities, but I request that my child's image not be singled out or featured in any promotional materials.

Please submit this form electronically at least 24 hours before the start of the basketball camp. Thank you for your understanding and cooperation.
